Procedure for accessing your medical record

An Act Respecting Health Services and Social Services as well as An Act respecting access to documents held by public bodies and the Protection of personal information and the Civil Code of Québec describe the ways that users and authorized third parties can access a medical record.

Request for a copy of a record or certificate

To get a copy of documents in your record or a copy of your entire medical record, you must fill out the following form :

Patients must fax a request to 514-593-2576 or send it to us by mail at the following address:

In certain cases, you may be asked to provide legal documents, such as a birth certificate, death certificate, will or power of attorney.

Please note that no medical information can be disclosed over the phone or by fax.

Fees

The first 20 sheets are free; however, the fees set out in the Access to Information Act apply to subsequent copies.
