A Unique Way of Providing Nursing Care
In 2003, nurses from all the nursing units
participated in defining and adopting a unique way of providing
nursing care, adapted to the patient's needs and to the realities
of the 21st century: concerted care.
Concerted Care
In this type of nursing care, the pivot
nurse (the nurse assigned to the patient) works closely with the
team, the patient and his/her family in order to respond
effectively and efficiently to the patient's needs.
The pivot nurse ensures that the proper people provide
appropriate and timely care. During his/her shift,
the pivot nurse is entirely responsible for the care
of one or more patients, depending on the intensity of care
required, in collaboration with the other members of the care
team and the interdisciplinary team. The care team is composed
of pivot nurses, orderlies working with one or
more pivot nurses and collaborators from the
interdisciplinary team. The pivot nurse is assigned to
patients according to the intensity of care required.
The Pivot Nurse:

Writes or modifies the therapeutic care
plan

Coordinates the interdisciplinary intervention
plan

Provides the care that requires his/her skills and
delegates others to an orderly (or another caregiver) while
ensuring supervision

Manages the episode of care of patients for whom
he/she is responsible

Ensures a high quality of care

Interacts with all the members of the
interdisciplinary team and the partners involved in patient
care

Communicates with the patient and
his/her family through active listening, teaching and support
to help them take part in the care plan
The Orderly:

Provides care according to the work plan kept up
to date by the pivot nurses

Informs the pivot nurse of any patient
problems and keeps him/her up to date on the work plan's
progress
