The liaison nurse


Supervised by the coordinator of nursing quality, infection control and product evaluation, the liaison nurse ensures continuity of care for patients of the Institute with resources within the network. The objective is to optimize patients' quick return to their environment.

The liaison nurse develops and consolidates relationships between the Institute and the CLSCs, referring hospitals, convalescent homes and community resources.

The liaison nurse refers patients of the Institute to the appropriate resources based on their identified needs, to ensure care follow-up. She acts as a consultant for her their colleagues and the multidisciplinary team, and assists with the distribution and updating of protocols and care methods among the Institute's external partners.
