The Heart Failure Clinic
The clinic offers follow-up services for heart
failure patients, mainly of the functional class NYHA III-IV. It is
supported by a medical team under the direction of Dr. Anique
Ducharme, and by the nursing coordinator of the clinic, three
nurse clinicians, a nurse practitioner, a pharmacist, a
dietician, a secretary and a desk clerc.
Our objective is to offer our patients the best possible
pharmacological treatment, clinical stability and an improved
quality of life. For all these activities, it is necessary to
underline the importance of telephone follow-ups. Services offered
by the Heart Failure Clinic make it possible to decrease the
number of admissions and visits to the Institute's emergency
unit.
Thanks to our close cooperation with the interdisciplinary team,
our patients benefit the best possible follow-up. We refer our
patients to services such as social services, psychosomatic
and CLSC, which reflects our constant pursuit of continuity of care
for our patients, in a spirit of interdisciplinary
collaboration.
