The mission of the Montreal Heart Institute is to provide specialized cardiology services to the entire province of Quebec.

To refer a patient, please send a cardiology consultation request by fax to 514-593-2567 or by mail to the attention of the Intake Service - Outpatient Clinic. The request must be signed by a doctor and contain the following information:

Patient information:

• Last and first name
• Date of birth
• Last and first name of the person's mother
• Health insurance number
• Full address
• Phone number
• Diagnoses and information related to the cardiology consultation request

Doctor's information:

• Doctor's signature
• License number
• Date

Consultation requests are assessed by a cardiologist.
Patients whose conditions require particular attention are contacted as soon as possible.
