Diabetic patients follow-up


Opened in September 2006 as a 2-year project, the diabetes clinic offers a 16-week follow-up to diabetic cardiac patients after their hospital discharge. The nurse clinician, the internists and the nutritionist join their efforts to achieve the following goals and objectives:

  • To develop and apply a specific care and teaching program for diabetic cardiac patients
  • To start an insulin treatment and ensure a short term follow-up to cardiac patients with acute unbalanced diabetes
  • To identify and analyse specific needs for those patients
  • To increase professional expertise
  • To optimize care and treatment
  • To rapidly detect abnormal glycaemia and intervene promptly
  • To ensure continuity of interdisciplinary care from hospitalisation to external follow-up
