Opened in September 2006 as a 2-year project, the diabetes clinic offers a 16-week follow-up to diabetic cardiac patients after their hospital discharge. The nurse clinician, the internists and the nutritionist join their efforts to achieve the following goals and objectives:
To develop and apply a specific care and teaching program for diabetic cardiac patients
To start an insulin treatment and ensure a short term follow-up to cardiac patients with acute unbalanced diabetes
To identify and analyse specific needs for those patients
To increase professional expertise
To optimize care and treatment
To rapidly detect abnormal glycaemia and intervene promptly
To ensure continuity of interdisciplinary care from hospitalisation to external follow-up
