Antibacterial prophylaxis for theprevention of endocarditis

1 - Dental procedures/oropharyngeal and respiratory
Standard general prophylaxis	Amoxicillin	2g PO 1 hour before the procedure
PO impossible	Ampicillin	2g IM/IV in the 30 min. prior to the procedure
Allergy to penicillins	Clindamycin or	600mg PO 1 hour before the procedure
Allergy to penicillins	Cephalexin** or	2g PO 1 hour before the procedure
Allergy to penicillins	Azithromycin or Clarithromycin	500mg PO 1 hour before the procedure
Allergy to penicillins and PO impossible	Clindamycin or	600mg IV in the 30 min. prior to the procedure
Allergy to penicillins and PO impossible	Cefazolin**	1g IM/IV in the 30 min. prior to the procedure
2 - Genitourinary/gastrointestinal procedures
High-risk patients	Ampicillin + Gentamicin	Ampicillin 2g IM/IV plus gentamicin 1.5 mg/kg (maximum 120 mg) in the 30 min. prior to the procedure AND ampicillin 1g IM/IV OR amoxicillin 1g PO 6 hours later
High-risk patients allergic to penicillins	Vancomycin + Gentamicin	Vancomycin 1g IV plus gentamicin 1.5 mg/kg IV (maximum 120mg); perfusions must be completed in the 30 min. prior to the procedure
Moderate-risk patients	Amoxicillin or Ampicillin	Amoxicillin 2g PO 1 hour before the procedure OR ampicillin 2g IM/IV in the 30 min. prior to the procedure
Moderate -risk patients allergic to penicillins	Vancomycin	Vancomycin 1g IV; the perfusion must be completed in the 30 min. prior to the procedure

** Cephalosporins should not be used if the allergy to penicillins displays itself as anaphylactic shock, angioedema or urticaria.
