Your medical file, we care



Your care at the Montréal Heart Institute is carefully charted in the medical record (usually referred to as the Health Record).


It is charted according to the Law on the Health Services and the Social Services and according to professional standards that bind the various health professionals.

The Law on the Health Services and the Social Services as well as the Law on access with the documents of the public organizations and the protection of the personal information and the Civil code protect the access of the Medical Health Record.

Access to the medical file

  • No medical information can be transmitted by phone or by fax.
  • No request by fax will be accepted.

To reach the information contained with a medical file, you must produce a written request and address this one to the MHI Medical Records Service.

The request must contain the following information:

  • Your name and first name
  • Your date of birth
  • Your file number at the Montréal Heart Institute (if available)
  • Your health insurance number
  • The name and first name of your mother
  • The nature of the information necessary as well as the date for which need some
  • Your signature
  • The address to send the documents

Expenses are required for the copies of files exceeding more than 20 pages.
